Criminal background check
Prior employment verification
Credentials check for licensure
I-9 compliance (E-Verify)
Voluntary Mental Health Check
At least two satisfactory relevant professional references (if applicable)
Copy and verifiable BLS and any additional certifications as required by designated areas
Minimum of one year of relevant or related experience
Acceptable background verifications including OIG, OFAC, SAMS, and Criminal
Complete immunization record (MMR, Hepatitis B, TDAP, Varicella, and other immunization records upon request)
National Sex Offender Search
Drug screening

Current physical examination within last year
